Hi Staplesei,
This is not a problem at all, many people forget some details on their Security Questionnaire. You should contact the Diplomatic Security Office if they left you with contact details. Otherwise, contact HR (Daphne Featherstone) at the State Department and let her know that you forgot to add an additional alias/name to the e-Quip Security Clearance form. She will put you in contact with DSS and they will make an update. It's easy, and as long as you don't ignore it this will not be an issue.

We are waiting for security clearances etc. right now. My husband was adopted when he was around 10, he took his adopted father's name at that time.
Due to the fact that he has not used this name for about 30 or more years, he forgot to add this name where it asks for any other names he has used.
We think his birth certificate says the name he has always gone by.
What do you think we should do at this point? Can we send in something to explain this mistake, or do we wait and see if it comes up?
